As most people are aware now the CDC expanded eligibility for an additional booster dose for certain individuals who may be at higher risk of severe outcomes from COVID-19. People over the age of 50 can now get an additional booster 4 months after their prior dose to increase their protection further. This second booster is not currently required for someone over 50 to be “up-to-date” but it is being offered in anticipation of possible future increases in cases due to potential new variants in the coming months. If you are over 50 or considered moderately to severely immunocompromised* and it has been 4 months since your first Moderna or Pfizer booster or 2 months since their last J&J shot you are eligible for another vaccination. Here are safer ways to celebrate the holidays:

Generally:

  • Protect those not yet eligible for vaccination such as young children by getting yourself and other eligible people around them vaccinated.
  • Wear well-fitting masks over your nose and mouth if you are in public indoor settings if you are not fully vaccinated.
    • Even those who are fully vaccinated should wear a mask in public indoor settings in communities with substantial to high transmission.
      • Outdoors is safer than indoors.
    • Avoid crowded, poorly ventilated spaces.
    • If you are sick or have symptoms, don’t host or attend a gathering.
    • Get tested if you have symptoms of COVID-19 or have a close contact with someone who has COVID-19.

If you are considering traveling for a holiday or event, visit CDC’s Travel page to help you decide what is best for you and your family. CDC still recommends delaying travel until you are fully vaccinated.

Special considerations:

  • People who have a condition or are taking medications that weaken their immune system may not be fully protected even if they are fully vaccinated and have received an additional dose. They should continue to take all precautions recommended for unvaccinated people, including wearing a well-fitted mask, until advised otherwise by their healthcare provider.
  • You might choose to wear a mask regardless of the level of transmission if a member of your household has a weakened immune system, is at increased risk for severe disease, or is unvaccinated.
  • If you are gathering with a group of people from multiple households and potentially from different parts of the country, you could consider additional precautions (e.g., avoiding crowded indoor spaces before travel, taking a test) in advance of gathering to further reduce risk.
  • Do NOT put a mask on children younger than 2 years old.

By working together, we can enjoy safer holidays, travel, and protect our own health as well as the health of our family and friends.

Federal officials have recently announced that they will recommend booster shots for all previously vaccinated Americans. They have said they will likely start with long-term care residents and staff along with others over 65  and move forward from there. Although we expect this to be carried out in a similar fashion to the first rounds, we have no further information at this time. Once we know more details we will share them with the community.

We are happy to announce that visitation in the Skilled Nursing and Assisted Living settings will reopen effective 6/19/21.

All visitors will need to come in the appropriate entrance (Security/Employee Entrance at MKV or Main Entrance to Assisted Living at MKV) and the Main Entrances at both Skilled Nursing and Assisted Living at TKO.

Visitors will no longer need to make appointments effective 6/19/21.

There are no limits on the number of visitors that come, but we ask for all to use their best judgment.

All visitors are required to go through an electronic screening process upon arrival and wear the screening badge that is produced during their visit.

All visitors must adhere to the following:

Core Principles of COVID-19 Infection Prevention

  • Screening of those who enter the facility for signs and symptoms of COVID-19 (e.g., temperature checks, questions about and observations of signs or symptoms), and denial of entry of those with signs or symptoms or those who have had close contact with someone with COVID-19 infection in the prior 14 days (regardless of the visitor’s vaccination status);
  • Hand hygiene (use of alcohol-based hand rub is preferred);
  • Face covering or mask (covering mouth and nose);
  • Social distancing at least six feet between persons;
  • Instructional signage throughout the facility and proper visitor education on COVID-19 signs and symptoms, infection control precautions, other applicable facility practices (e.g., use of face covering or mask, specified entries, exits and routes to designated areas, hand hygiene);
  • Cleaning and disinfecting high-frequency touched surfaces in the facility often, and designated visitation areas after each visit;
  • Appropriate staff use of Personal Protective Equipment (PPE);
  • Effective cohorting of residents (e.g., separate areas dedicated to COVID-19 care);
  • Resident and staff testing conducted as required

These core principles are consistent with the CDC guidance and should be adhered to at all times so that visitation(s) may continue.

As COVID-19 restrictions are continuing to be lifted, we are able to reopen access to some of the programs and services that you have all missed over the past year.   Below you will find a list of accessible locations with dates that they will open.

Mane Street Salon:  We will be able to open to Independent Living residents beginning the week of June 28th.  You can schedule an appointment with Helen Hutchison, Cherie Alexander or China Wise by calling 513.782.2432.

ServUS Lab: The ServUS Lab will be open and fully operational for Independent Living residents beginning July 12th

Chapel on the Green: The Main Street Chapel will be open for Independent Living beginning July 12th.  Chaplain Nancy Vilaboy-Allphin will be sending additional notices on upcoming programs and services. 

Bistro on the Green and Main Street MarketPlace: Both dining venues are expected to be completed in July/August with a grand opening to be scheduled after project completion.  Additional details will be provided as available.

Clinic: The clinic will continue to accessible by appointment only.  Appointments can be made by calling 513.782.2448.

Welcome Center: The Maple Knoll Village Welcome Center/Marketing Department’s doors are open Monday-Friday from 8am-4:30pm. 

Wellness Center: The Wellness Center is continuing to operate normally for residents.  We are currently not servicing non-residents at this time.

Cheers Pub and the Library: These locations are currently open 24/7 for Independent Living residents only.

We ask that unvaccinated residents continue to wear masks when in common spaces.  Again, we want to thank you for your patience and cooperation.  We look forward to seeing your smiling faces in the main building once again!

As you may have heard, Governor Dewine is expected to make changes to both SNF and ALU visitation orders on 6/18/21. Our staff will be anxiously awaiting those directives tomorrow morning and will notify residents and family members once all of the information has been received and processed. We are excited to share that the Governor’s health orders were rescinded today with the exception of Nursing Homes and Assisted Livings. Therefore the majority of changes only affect Independent Living residents and their space.

Effective immediately:

  • The Beecher gate can remain open at all times.
  • Residents will still not be able to go down Main Street due to construction.
  • Unvaccinated residents will be encouraged to wear masks in common spaces and will be required to at The Beecher Place Dining Room. 
  • The Kroger Shuttle bus will resume in the upcoming weeks with additional details to follow. 
  • Cheers can return to normal operation. 
  • The limit on the number of people at events no longer exists, which means that Independent Living Activities will begin to plan normal activity programs.
  • No changes to clinic operations at this time.
  • The Wellness Center will return to normal operations inside the exercise room for our IL residents on 6/7.  Group classes will occur in the Auditorium and will be announced through the Wellness Center. 
  • The Beecher Dining Room will return to normal operations for our IL residents on 6/7. Residents will be able to bring guests to the dining room beginning the week of 6/21.

Again, Skilled Nursing and Assisted Living areas are still under restricted access for outside guests.  Staff and contracted staff must wear masks at all times.  Thank you for your continued patience and support!

Resident Outings

If a resident who has been fully vaccinated goes out with a family member and has not had a known exposure during their outing they will not need to be tested or quarantined upon their return. Unvaccinated residents who go on an outing with visitors will still require weekly testing for 2 weeks upon return. All residents will continue to be monitored at least daily for signs and symptoms.

VISITATION CHANGES

This triggers an exception to indoor visitation for Skilled Nursing based on the current Ohio Department of Health order and indoor visitation must be suspended immediately as stated below :

When a new case of COVID-19 among residents or staff is identified, a nursing home should immediately begin outbreak testing and suspend all visitation until at least one round of facility-wide testing is completed. Visitation can resume based on the following criteria:

• If the first round of outbreak testing reveals no additional COVID-19 cases in other areas (e.g., units) of the nursing home, then visitation can resume for residents in areas/units with no COVID-19 cases. However, the nursing home should suspend visitation on the affected unit until the nursing home meets the criteria to discontinue outbreak testing.

• If the first round of outbreak testing reveals one or more additional COVID-19 cases in other areas/units of the nursing home (e.g., new cases in two or more units), then nursing homes should suspend visitation for all residents (vaccinated and unvaccinated), until the nursing home meets the criteria to discontinue outbreak testing.

We will update indoor visitation access after our first round of outbreak testing early next week.

Compassionate and end of life visits will still be permitted.

     The Ohio Department of Health issued an updated order for visitation which you can read in its entirety at coronavirus.ohio.gov. Go to the “Latest Developments” section for the full order. Some of the highlights are as follows:

  • Visits MUST be scheduled with the facility.
  • Visits are a minimum of 30 minutes. We will be offering visits that will be 45 minutes in duration.
  • Visits can be in the resident’s room if they are in a private room.
  • Only 2 visitors are permitted in for a visitation.

Please remember that the logistics to make this work require staff time. While we certainly want our residents to have visits, they must be scheduled in a fashion that enables us to meet protocol that we have to follow. For example, staff have to escort visitors to and from visitation areas as well as monitor visits for compliance with infection control and social distancing.

Update for Breese Manor & Bodmann Residents

As rules regarding visitation, quarantine, and vaccinations evolve we have fielded questions about taking a resident out of the building for non-medical reasons e.g. to a park, store, or restaurant.

To be clear, this would violate the current visitation order and we do not encourage this action. Positivity rates continue to be elevated in our county and other counties nearby and this can put the resident at risk for exposure regardless of vaccination status.

If a resident is taken out for a non-medical reason into any environment we have developed a process to minimize risk to the resident and other staff and residents. If the returning resident is unvaccinated they will be subject to quarantine for 14 days and tested at least once a week for two weeks. If the returning resident is vaccinated (14 days from the second dose) they will not require quarantine but will be tested at least once a week for two weeks.  

For medically required appointments or hospital visits less than 24 hours in a supervised environment, there will be no changes to resident care.

We continue to monitor all residents at least daily for signs and symptoms.

We are happy to pass along this announcement received from Leading Age today.  Fourteen-day quarantine is no longer required for incoming residents or transfers unless there has been prolonged exposure to COVID.  At this time, visitation remains the same although we do expect further guidance in April.  Call 513.782.2686 to schedule.

New admissions no longer require quarantining:
Implications for nursing and
assisted living facilities

On March 10, the Centers for Disease Control & Prevention (CDC) updated select healthcare infection prevention and control recommendations in response to COVID-19 vaccination. The Ohio Department of Health (ODH) has received approval by the Centers for Medicare & Medicaid Service (CMS) that nursing homes and assisted living facilities can follow these recommendations. ODH confirmed this applies to all healthcare personnel (HCP) while at work and all patients and residents while they are being cared for in a healthcare setting.

Quarantine is no longer recommended for residents who are being admitted to a post-acute care facility if they are fully vaccinated and have not had prolonged close contact with someone with SARS-CoV-2 infection in the prior 14 days. This implies that residents who leave the facility (and do not have similar prolonged contact) also do not need to be quarantined upon return. Facilities are urged to have a person-centered approach to reviewing each resident’s entry/return to the facility.

Fully vaccinated residents should continue to quarantine following prolonged close contact (within 6 feet for a cumulative total of 15 minutes or more over a 24-hour period) with someone with SARS-CoV-2 infection. This is due to limited information about vaccine effectiveness in this population, the higher risk of severe disease and death, and challenges with physical distancing in healthcare settings

Fully vaccinated healthcare professionals (HCP) with higher-risk exposures who are asymptomatic do not need to be restricted from work for 14 days following their exposure. 

The CDC recommends quarantining if the HCP that had a high-risk exposure is immunocompromised. HCP who have traveled should continue to follow CDC travel recommendations and requirements, including restriction from work, when recommended for any traveler.

Nursing Home Visitation Guidance Update

On Wednesday, the Centers for Medicare & Medicaid Services (CMS) released revised visitation guidance for nursing homes. The guidance allows nursing homes to expand visitation options in a safe and responsible manner, regardless of vaccination status of the resident or visitor.  Although CMS has updated guidance for nursing home visitation, Ohio’s sixth amended order remains in effect which we are required to follow.  This means that visitation is still restricted except for end of life and compassionate care visits. 

We continue to advocate for changes in visitation in conjunction with LeadingAge Ohio. As we receive guidance we will continue to provide updates.  

In addition, we are pleased to share that 98% of our residents campus wide have been vaccinated.  We have had a total of 70% of our staff who have received the vaccine.  Please note that we are unable to mandate the vaccine for employees while the FDA still categorizes it as emergency use.  Until the FDA gives the vaccine full approval, we will continue to educate and highly encourage our employees to receive it.

Skilled Nursing Visitation Reminder:

Compassionate care visits, except for end of life visits, are held in a designated visiting area on the first floor unless other arrangements have been made according to resident condition.

Beginning Monday, March 1, the visitor entrance procedure will change. Guests with an appointment to see their loved one via first-floor visitation arrangements will be screened to enter by the visitation attendant at the business office door, located at the western end of the building; closest to Springfield Pike.  There will be a doorbell set up for entry to the building. We ask our guests to please not arrive more than 5 minutes ahead of the appointment time, as we do not have an indoor waiting area.

Nearly one year ago, we started to take preventive measures to combat the COVID-19 pandemic. Twenty-Twenty was an emotional roller coaster for many of us, but we faced it together. Maple Knoll Communities has been notified that Walgreens will now be able to vaccinate all Independent Living residents. Residents will be receiving the Pfizer vaccine which has proven to be greater than 94% effective in preventing COVID-19. You will receive the vaccination in two doses. The first will occur on January 22nd, 2021 with the second occurring 21 days later.

At this time we are trying to gauge the interest that Independent Living residents have on receiving the vaccine on our campus. While we know it is high, we want to ensure that we are providing accurate numbers on the amount of individuals needing vaccinations. Independent Living residents who would like to receive the vaccine please let us know by calling 513.909.7935, responding via the app or emailing us at info@mkcommunities.org. They must provide this notification to us by 5pm on Monday, January 18th.

Again, the vaccination will occur between the hours of 8am and 4pm on Friday, January 22nd. We will be sharing additional information, such as the specific timeslot for your neighborhood, as well as the consent forms on January 19th. The vaccination will occur on the campus of Maple Knoll Village and shuttle service will be provided for those needing transportation. We wanted to provide you with an update on what is occurring with vaccinations and restrictions on our campus. Please know that Maple Knoll Communities has been working closely with Walgreens, our national pharmacy provider, for the past two months preparing for the availability of a COVID-19 vaccine. Yesterday, our Skilled Nursing residents and staff received their second dose of the COVID-19 vaccine by Walgreens. We are also pleased to share that we were able to have our Assisted Living residents and staff receive their first vaccination yesterday as well. They will receive their second vaccination on February 2nd. This is one step closer to having all of our residents vaccinated. We are also working to get staff vaccinated so that we can safely bring programming and services back for all residents to enjoy. We appreciate your patience as we work through this.

Our Infection Preventionist continues to push for our Independent Living residents to receive the vaccination yet we do not have answers on when this will occur. As previously shared, we are at the mercy of Governor Dewine’s orders and the vaccine availability through Walgreens. Also, a reminder that as we balance Maple Knoll’s desire to be transparent with our need to protect the privacy of the residents we serve, we are unable to release information on specific test results for Independent Living residents. We will continue to share positive cases of staff, skilled and assisted residents and we will work with the local health department to assist with contract tracing and will notify you if you should be quarantined. Governor Dewine Update Regarding Vaccines for Group 1B

Governor Dewine indicated yesterday the details about when the next group of vaccine recipients may receive a vaccine:

Age 80 and up will begin vaccinations on January 19th;

Age 75 and older will begin on Jan 25th;

Age 70 and older will begin Jan 31st;

Age 65 and older will begin Feb 8th.

Individuals with cerebral palsy, congenital heart disease, Type 1 DM, epilepsy, severe asthma; sickle cell anemia and other diseases will begin Jan 25th.

The Ohio Department of Health has identified more than 1,700 providers to administer vaccines to these groups.

Providers will be required to list whether an appointment is required.

ODH plans to post a list of sites administering vaccines in each of Ohio’s 88 counties next week.

Locations will be listed online at coronavirus.ohio.gov starting next Thursday.

Happy holidays to all of our residents!

We have been getting many calls about vaccinations for Independent Living. At this time, the state is only allocating vaccinations for skilled nursing residents and staff. Assisted Living is expected to be next in line. We are partnered with Walgreens for vaccinations and have no vaccine available on site. We will update our campus residents when we receive guidance on Independent Living.

WHAT YOU NEED TO KNOW BEFORE YOU GET THIS VACCINE?

WHAT IS COVID-19?

COVID-19 disease is caused by a coronavirus called SARS-CoV-2. This type of coronavirus has not been seen before. You can get COVID-19 through contact with another person who has the virus. It is predominantly a respiratory illness that can affect other organs. People with COVID-19 have had a wide range of symptoms reported, ranging from mild symptoms to severe illness. Symptoms may appear 2 to 14 days after exposure to the virus. Symptoms may include: fever or chills; cough; shortness of breath; fatigue; muscle or body aches; headache; new loss of taste or smell; sore throat; congestion or runny nose; nausea or vomiting; diarrhea.

WHAT IS THE PFIZER-BIONTECH COVID-19 VACCINE?

The Pfizer-BioNTech COVID-19 Vaccine is an unapproved vaccine that may prevent COVID-19. There is no FDA-approved vaccine to prevent COVID-19.

The FDA has authorized the emergency use of the Pfizer-BioNTech COVID-19 Vaccine to prevent COVID-19 in individuals 16 years of age and older under an Emergency Use Authorization (EUA).

WHAT SHOULD YOU MENTION TO YOUR VACCINATION PROVIDER BEFORE YOU GET THE PFIZER-BIONTECH COVID-19 VACCINE?

Tell the vaccination provider about all of your medical conditions, including if you:

  • have any allergies
  • have a fever
  • have a bleeding disorder or are on a blood thinner
  • are immunocompromised or are on a medicine that affects your immune system
  • are pregnant or plan to become pregnant
  • are breastfeeding
  • have received another COVID-19 vaccine

WHO SHOULD GET THE PFIZER-BIONTECH COVID-19 VACCINE?

FDA has authorized the emergency use of the Pfizer-BioNTech COVID-19 Vaccine in individuals 16 years of age and older.

WHO SHOULD NOT GET THE PFIZER-BIONTECH COVID-19 VACCINE?

You should not get the Pfizer-BioNTech COVID-19 Vaccine if you:

  • had a severe allergic reaction after a previous dose of this vaccine
  • had a severe allergic reaction to any ingredient of this vaccine

WHAT ARE THE INGREDIENTS IN THE PFIZER-BIONTECH COVID-19 VACCINE?

The Pfizer BioNTech COVID-19 Vaccine includes the following ingredients: mRNA, lipids ((4-hydroxybutyl)azanediyl)bis(hexane-6,1-diyl)bis(2-hexyldecanoate), 2 [(polyethylene glycol)-2000]-N,N-ditetradecylacetamide, 1,2-Distearoyl-sn-glycero-3- phosphocholine, and cholesterol), potassium chloride, monobasic potassium phosphate, sodium chloride, dibasic sodium phosphate dihydrate, and sucrose.

HOW IS THE PFIZER-BIONTECH COVID-19 VACCINE GIVEN?

The Pfizer-BioNTech COVID-19 Vaccine will be given to you as an injection into the muscle.

The Pfizer-BioNTech COVID-19 Vaccine vaccination series is 2 doses given 3 weeks apart.

If you receive one dose of the Pfizer-BioNTech COVID-19 Vaccine, you should receive a second dose of this same vaccine 3 weeks later to complete the vaccination series.

HAS THE PFIZER-BIONTECH COVID-19 VACCINE BEEN USED BEFORE?

The Pfizer-BioNTech COVID-19 Vaccine is an unapproved vaccine. In clinical trials, approximately 20,000 individuals 16 years of age and older have received at least 1 dose of the Pfizer-BioNTech COVID-19 Vaccine.

WHAT ARE THE BENEFITS OF THE PFIZER-BIONTECH COVID-19 VACCINE?

In an ongoing clinical trial, the Pfizer-BioNTech COVID-19 Vaccine has been shown to prevent COVID-19 following 2 doses given 3 weeks apart. The duration of protection against COVID-19 is currently unknown.

WHAT ARE THE RISKS OF THE PFIZER-BIONTECH COVID-19 VACCINE?

Side effects that have been reported with the Pfizer-BioNTech COVID-19 Vaccine include:

  • injection site pain
  • tiredness
  • headache
  • muscle pain
  • chills
  • joint pain
  • fever
  • injection site swelling
  • injection site redness
  • nausea
  • feeling unwell
  • swollen lymph nodes (lymphadenopathy)

There is a remote chance that the Pfizer-BioNTech COVID-19 Vaccine could cause a severe allergic reaction. A severe allergic reaction would usually occur within a few minutes to one hour after getting a dose of the Pfizer-BioNTech COVID-19 Vaccine. Signs of a severe allergic reaction can include:

  • Difficulty breathing
  • Swelling of your face and throat
  • A fast heartbeat
  • A bad rash all over your body
  • Dizziness and weakness

These may not be all the possible side effects of the Pfizer-BioNTech COVID-19 Vaccine. Serious and unexpected side effects may occur. Pfizer-BioNTech COVID-19 Vaccine is still being studied in clinical trials.

If you experience a severe allergic reaction, call 9-1-1, or go to the nearest hospital.

Currently, there is no approved alternative vaccine available for prevention of COVID-19. FDA may allow the emergency use of other vaccines to prevent COVID-19.

WHAT IS AN EMERGENCY USE AUTHORIZATION (EUA)?

The United States FDA has made the Pfizer-BioNTech COVID-19 Vaccine available under an emergency access mechanism called an EUA. The EUA is supported by a Secretary of Health and Human Services (HHS) declaration that circumstances exist to justify the emergency use of drugs and biological products during the COVID-19 pandemic.

The Pfizer-BioNTech COVID-19 Vaccine has not undergone the same type of review as an FDA-approved or cleared product. FDA may issue an EUA when certain criteria are met, which includes that there are no adequate, approved, available alternatives. In addition, the FDA decision is based on the totality of scientific evidence available showing that the product may be effective to prevent COVID-19 during the COVID-19 pandemic and that the known and potential benefits of the product outweigh the known and potential risks of the product. All of these criteria must be met to allow for the product to be used in the treatment of patients during the COVID-19 pandemic.

The EUA for the Pfizer-BioNTech COVID-19 Vaccine is in effect for the duration of the COVID-19 EUA declaration justifying emergency use of these products, unless terminated or revoked (after which the products may no longer be used).

Although none of our assisted living residents have tested positive to date we still have to prepare for that possibility. We have recently created a COVID isolation unit in Bodmann specifically to care for any of our residents who test positive for COVID. This is to reduce the possibility of spread among our community and to provide necessary nursing care in these instances. So moving forward any Assisted Living Resident in Breese Manor will be moved to this unit upon receiving a positive COVID result and will stay on the unit for the duration of their isolation time as determined by our Infection Preventionist under the direction of the Medical Director.

Covid-19 is surging all around us!  In an attempt to mitigate spread of the virus on our campus, we are reverting back to our practices that were in place early on in the pandemic.  These changes are targeted and temporary until such time when our Pandemic Response Committee determines they are no longer necessary.  The departmental changes are as follows:

Foodservice: The Beecher dining room will close at end of service on Monday, November 23rd.  You will receive information from Lisa Lantry on the process that will begin on Tuesday, November 24th.

Independent Living Activities:  Effective Monday, November 23rd, all scheduled activities will cease until further notice.  This includes all groups that meet to play cards, games or other activities that are not on the activity department’s calendar. There can be no gatherings of any residents in any common space on our campus.  This includes the Manor House, Kensington Place “L” Common room and all Beecher Place living rooms. Just a reminder, there are books and puzzles you may borrow from Sandy’s office and enjoy in your individual unit.  Please refer any questions to Jim Hoeh at 513.782.2462.

Housekeeping Services:  Effective Wednesday, November 25th, we will be temporarily suspending housekeeping services to Independent Living until further notice.  For those residents who receive linen and toilet paper, we will continue providing linen but will only be making beds for residents who require assistance due to healthcare concerns.  You must have a mask on and stay in another room while your bed is made.  Please contact Jim Hoeh at 513.782.2462 if you need the bed making service.  We will be limiting toilet paper to a maximum of 2 rolls per home each week.

Maintenance and Transportation:  Effective immediately, we will not be accepting any non-emergency Maintenance and IT work orders. In addition, we will not be accepting new appointments for the MKV Chauffeur.  We will honor appointments already scheduled through the end of day on 11/24.  Requests for medical transportation should be directed to the Service Desk at 513.782.2500 in order to be scheduled with Maple Knoll Outreach Services Transportation Department.  Shuttle on demand will still be available for transportation to Outpatient Therapy and clinic appointments.  Meal delivery will occur during set hours each day.

Outpatient Therapy:  Outpatient therapy operate in Wellness Center.

Clinic:  The clinic will remain open.  In person appointments will be made at the discretion of the UC physicians and telehealth visits will be used whenever possible.

We must have a collective commitment to reducing the spread of this virus until we are able to receive an effective vaccine.  Thank you for your understanding and cooperation.

Updated Maple Knoll Communities’ Visitation Policy for Skilled Nursing and Assisted Living

To allow indoor visitation is a decision our facilities make on a weekly basis. This decision is based on three factors : the county positivity rate, our current color designation on the Ohio Public health advisory map, and whether or not we have had a staff or resident recently test positive for COVID. Visitation will be placed on hold in the event of any staff or resident testing positive for COVID in the previous two weeks, the respective county being “red” or “purple” in color on the Ohio Public Health Advisory System map; and/or the county positivity rate being 5% or greater.

However, CDC and federal guidelines now require increased levels of personal protective equipment by staff and nasal swab COVID testing of residents at least weekly for a minimum of 14 days. Resident Visits at Maple Knoll Village- Updated September 25th, 2020

At the Ohio Governor’s press conference yesterday, Director of Aging Ursel McElroy announced that the DeWine Administration will allow indoor visitation for residents of Ohio’s nursing homes and assisted living beginning October 12. “The prolonged loss of connection is critical,” said the Director, noting that they had consulted with aging advocates, including LeadingAge Ohio, to develop the guidelines for indoor visitation. “Indoor visitation does not signal that we can be less cautious,” she said, but rather we should be “more vigilant” in taking steps to protect residents. In addition to masks and handwashing, visitors should also be attuned to symptoms in themselves, their families and other people with whom they have contact.

As with outdoor visitation, visits will be required to be scheduled ahead of time, and visitors will undergo a health screening before entering the building. Facilities will keep a log of visitors. Changes include a visitation time limit of 30 minutes and a requirement that facilities provide masks to residents’ visitors. While there is no minimum age limit defined, all visitors must be able to wear a mask and follow social distancing requirements. In line with CMS guidelines, “Facilities will not restrict visitation without a reasonable clinical or safety cause.” The state will also launch a coronavirus dashboard, which will allow families/others to access a facility’s visitation information, including whether visitation is currently available, visiting hours and contact information for scheduling a visit. The dashboard will be posted to www.coronavirus.ohio.gov, and will be launched when visitation begins, on October 12.

Maple Knoll Village is currently developing a plan to provide indoor visits to loved ones in Skilled Nursing and Assisted Living while taking into consideration necessary precautions for their safety and wellbeing. Here at Maple Knoll Communities, we are closely monitoring the COVID-19 situation and, in accordance with The Ohio Department of Health, World Health Organization and Center for Disease Control,, are continuously evaluating our policies, procedures and taking extra precautions. Because the health and well-being of our residents and employees remains our first priority, we continue to make extensive preparations, realizing this situation has an unpredictable path. Maple Knoll Communities’ Corporate Infection Preventionist will be coordinating all precautions, screenings, supplies, education, updates and communication related to the virus.

On July 20, 2020, Interim Director of Health Lance Himes signed a Director’s Order continuing to restrict access to Ohio’s long-term care facilities, including both nursing homes and assisted living centers.

During a press briefing (58 minute mark) on Thursday, September 17, 2020, Governor DeWine stated:
“If you have a loved one, and you cannot get into a nursing home, that is the nursing home that is stopping you from going in. It is not the Health Department.”


This is incorrect. LeadingAge Ohio has advocated for months for the reopening of safe indoor visitation, and is currently working with association partners and the DeWine Administration on guidance for that reopening. Neither the Governor nor the Ohio Department of Health have given providers guidance or permission to resume indoor visitation, and the Administration later clarified that indoor visitation has not yet been reopened by the state.


Per the still-active July 20 Director’s Order: 

Nursing facilities and assisted living centers must continue to restrict access and are not permitted to offer indoor visitation.

  • The order applies to: nursing facilities, residential care facilities (RCFs) which includes assisted living, and ICF/IIDs.
  • Access continues to be restricted only to personnel necessary to the operation of the Homes.
  • No visitors of residents are permitted to enter any Home, except for end-of-life situations.

Nursing facilities and assisted living centers may continue to offer outdoor visitation.

  • Providers are asked to prepare a comprehensive plan before reopening outdoor visitation.
  • Outdoor visitation policies must include visitor screening, mask requirements, adequate staffing, and more.
  • Visits at nursing homes/RCFs shall not exceed one hour in length, nor are more than three visitors allowed per resident per visit.

LeadingAge Ohio continues to work with association partners and the Administration on a plan to reopen indoor visitation

On Monday, August 10th, 2020 at 3:30 p.m., Maple Knoll Village was notified of a COVID-19 positive test result on an employee in Skilled Nursing. The employee has been removed from duty until they have been cleared to return by the Infection Preventionist in coordination with the local Health Department. The local Health Department has been notified and the employee had minimal exposure in the facility.  All employees are screened daily for symptoms of COVID-19. In addition, all residents in assisted living and nursing are screened daily for signs and symptoms of COVID-19. The employee has been removed from duty until they have been cleared to return by the Infection Preventionist in coordination with the Local Health department. The local Health Department has been notified and the employee had minimal exposure in the facility. All employees are screened daily for symptoms of COVID-19. In addition, all residents in assisted living and nursing are screened daily for signs and symptoms of COVID-19. We are pleased to announce that the Governor and the Ohio Department of Health have announced the Responsible RestartOhio which will positively impact our residents residing in Assisted Living. Effective Monday 6/8/2020 visitors will be permitted for outdoor patio visitation in Breese Manor Assisted Living only.

Please click on Educational Information for all residents and family members to access additional requirements and a one time waiver. The guidelines set forth by the state require that family members must be screened for a temperature below 100.00 and symptom screening for COVID-19. Hand Sanitizer will be available and used upon arrival for visitation. Visitors agree to no physical contact and abide by 6 foot social / physical distancing requirements. There can be no personal contact. Drop off of items in front entry remains the same. Visitors must wear a mask covering the nose and mouth throughout the visit. The resident must also wear a mask. Visits will be in structured settings designed for social distancing and monitored to address any issues of misuse of facial coverings or lack of cooperation with social distancing. No pets will be allowed on the patios for visitation.

Unfortunately, failure to abide by the outdoor visitation policies/procedures may result in suspension of visits until compliance is achieved or at the discretion of the Facility.

Please call the Activity Coordinator phone number at (513) 782-4323 to make an appointment for two (2) visitors only, at one time, per resident in 30 minute increments between 10am -12noon and 1 pm – 4pm being the last appointment, Monday through Saturday. We have 4 areas on the patio to the left of the Breese Manor entrance. Please do not move the chairs/benches from designated areas. If these fill there will be 3 areas on the patio just outside of the main dining room.

On Wednesday, June 3rd, 2020 at 1:00 p.m., Maple Knoll Village was notified of a COVID-19 positive test result on an employee in Dietary. The employee has not been on our campus in over 14 days and is not felt to be a concern for residents or staff. The local Health Department has been notified and the said employee will not report to campus until medical clearance to return to duty has been received. All employees are screened daily for symptoms of COVID-19. In addition, all residents in assisted living and nursing are screened daily for signs and symptoms of COVID-19. We are continuing to monitor the progression of the COVID-19 virus and wanted to provide you with an update following Governor Mike DeWine’s recent recommendations as well as updates for our campus specifically. These include:

  • We are pleased to share that as of today, June 2nd, 2020, we have not had a single positive resident COVID-19 case at Maple Knoll Village.
  • Beginning June 8th, we will be able to offer expanded outdoor family visits in Assisted Living. Additional information will be forthcoming such as screening procedures, usage of masks and social distancing. Skilled Nursing facilities are still required to restrict access to their buildings which means no visitors at this time. We are working closely with the Ohio Department of Health and CMS to follow their guidelines on when we are legally and safely allowed to open. At this time there is not a date for this to occur.
  • Common Spaces such as activity areas, wellness center, library, Cheers Pub, etc. remain closed at this time. Skilled Nursing and Assisted Living facilities will also remain on quarantine for the safety of those living in our communities. While many fitness centers have been allowed to open, we still fall under different regulations and guidelines as our Wellness Center is located in our health care building. Please note that all guidelines are being followed in conjunction with requirements by the Governor’s office.
  • Independent Living Activities Directors are working with the Pandemic Committee to determine what activities can be brought back in a modified manner to safely meet resident entertainment needs. Additional information will be provided in upcoming weeks.
  • We have been able to open our Beecher Dining Room. We will follow strict guidelines as recommended by The Ohio Department of Health and the Center for Disease Control. Shuttle services are being provided. Reservations are required and will need to be made by 4pm the day prior and can be made by calling 513.782.2418. Delivery options will still be available for those in need, as requested. Dining times are as follows:
  • Monday-Sunday Lunch Service: Seating available from 12:00pm until 1:30pm
  • Monday-Saturday Dinner Service: Seating available at 4:30pm, 5:00pm, 5:30pm and 6:00pm
  • Governor DeWine has mandated that beauty salons can open effective May 15th with new guidelines and restrictions. Unfortunately, due to the location of our Main Street and Looking Glass Salons, we just received updated information from The Ohio Department of Health stating that we will be unable to open to residents in any level of care at this time.
  • Independent Living patients that currently use the clinic and have one of our doctors as their Primary Care Physician may still call the clinic at 782.2448 and request a tele-health appointment if they need to speak to a doctor. Again, because of our clinic location we are not allowed to open the space at this time for Independent Living residents per The Ohio Department of Health. We are also working to provide our residents access to physicians’ main offices that are located outside of Maple Knoll’s campus. Information on outside office hours is available by calling the clinic.
  • Independent Living Housekeeping has resumed on a limited service basis as part of our Pandemic Housekeeping Service schedule.
  • Independent Living Residents MUST wear masks while an employee is in their home and also practice social distancing as much as possible. We also encourage all residents to wear masks when out in the community. Maple Knoll Village is providing one mask per person in Independent Living. You may request a mask by calling 513.909.7935.
  • We will continue to have all staff and necessary vendors wear medical masks while on Maple Knoll’s campus to protect our residents. Enhanced screening technology has been purchased for our location.
  • Our internal Pandemic Committee will continue to meet twice a week and provide daily reports from the Corporate Risk Manager to the Committee.

On Sunday, May 24th, 2020 at 10:00a.m., Maple Knoll Village was notified of a COVID-19 positive test result on an independent contracted home health care company employee who had spent time on private duty in Independent Living. The employee has been removed from duty by the Home Care company for a period of time of at least 14 days and until medical clearance to return to duty has been received. Maple Knoll Village is working with the organization to ensure that all protocol is being followed and residents possibly impacted have been directly contacted. All employees are screened daily for symptoms of COVID-19. In addition, all residents in assisted living and nursing are screened daily for signs and symptoms of COVID-19. In conjunction with the Governor’s recommendations on restaurant re-openings we have decided to reopen our Beecher Place Restaurant for Maple Knoll Village Independent Living Residents beginning on Tuesday, May 26th, 2020. Dining times will be as follows:

Monday-Sunday Lunch Service: Seating available from 12:00pm until 1:30pm

Monday-Saturday Dinner Service: Seating available at 4:30pm, 5:00pm, 5:30pm and 6:00pm

We will follow strict guidelines as recommended by The Ohio Department of Health and the Center for Disease Control. This will include capacity limited to 92 individuals at one time, assigned seating in groups of households and individuals, additional cleaning procedures and mandatory reservations. Households will be classified as couples or individuals living in the same home. Tables will be required to be a minimum of 6 feet apart. Guests will not be permitted at this time. Residents are asked to please respect our 6 feet of social distancing requirements by not gathering in the lobby or common spaces prior to or after your scheduled meal time. Reservations will be required to be made by 4pm the day prior and can be made by calling 513.782.2418.

Delivery options will still be available for those in need as requested. Thank you for your patience and cooperation as we work towards the common goal of providing a safe and pleasant dining experience for all residents.

  • Independent Living patients that currently use the clinic and have one of our doctors as their Primary Care Physician may still call the clinic at 782.2448 and request a tele-health appointment if they need to speak to a doctor.  Again, because of our clinic location we are not allowed to open the space at this time for Independent Living residents per The Ohio Department of Health.  We are also working with physicians to provide access to outside offices they have access too. 
  • Independent Living Housekeeping has resumed on a limited service basis as part of our Pandemic Housekeeping Service schedule. Residents that did not receive any housekeeping services prior to the pandemic are not affected. The resident’s assigned cleaning day each week will be the same as it was prior.   • Common Spaces such as activity areas, wellness center, library, and Cheers Pub remain closed at this time.  Skilled Nursing and Assisted Living facilities will also remain on quarantine for the safety of those living in our communities.

Unfortunately, due to the location of our Main Street and Looking Glass Salons, we are unable to open these to Independent Living residents at this time.  As restrictions are lifted we will notify Independent Living residents of potential usage of these areas.  Assisted Living and Skilled Nursing residents will be given access to the Salons in their specific building, in accordance with our quarantine efforts, with additional precautions such as one client at a time and fully sanitizing the work area in between resident appointments.
  • Maple Knoll Village will continue to operate as we have for the last few weeks in order to protect our residents.  This includes: continuing the delivering of meals and essential items, restriction on group activities and access to common spaces, screening of all essential employees and vendors who enter our facility and mandatory quarantine of residents in Assisted Living and Skilled Nursing.  We will also be assessing future access to our Main Street Café and Beecher Dining Room.

We are continuing to monitor the progression of the COVID-19 virus and wanted to provide you with an update following Governor Mike DeWine’s recent changes as well as updates for our campus specifically. These include:

  • We are pleased to share that as of today, April 29th, 2020, we have not had a single positive COVID case at Maple Knoll Village. This includes employees and residents. We also do not have any outstanding COVID tests for any MKV employee or resident at present.
  • On Monday, Governor DeWine updated Ohio on the plan called “Responsible Restart Ohio”, which includes a May 1st opening for healthcare procedures that do not require overnight stays; May 4th opening of manufacturing, construction distribution centers and general offices; May 12th Consumer Retail Services will open with restrictions.
  • Maple Knoll Village will continue to operate as we have for the last few weeks in order to protect our residents. This includes: continuation of delivering of meals and essential items, restriction on group activities and access to common spaces, screening of all essential employees and vendors who enter our facility, mandatory quarantine of residents in Assisted Living and Skilled Nursing and suggested stay at home order by recommendation of the Governor for residents in Independent Living.
  • We will continue to have all staff wear medical masks while on Maple Knoll’s campus to protect our residents. In addition, The Center for Disease Control issued the recommendation that citizens wear face coverings in public settings where other social distancing measures are difficult to maintain, especially in areas of significant community-based transmission.
  • Independent Living residents are asked to limit your exposure by only having necessary visitors and encouraging them to wear a mask.
  • Continuation of the meeting of an internal Pandemic Committee twice a week as well as daily reports by the Corporate Risk Manager to the Committee.

Maple Knoll COVID-19 Q&A

Since the start of the pandemic, Maple Knoll Communities has prioritized the safety and health of all residents and employees by taking precautionary measures in a variety of ways. It’s because of these efforts that, to date, we currently do not have any active reported cases of the Coronavirus at Maple Knoll Village and The Knolls of Oxford. We know keeping up with the latest protocols and mandates can be stressful as they seem to change nearly every day. Our main priority is to keep you safe and healthy. Here are some responses to questions our residents might have regarding the virus:

What if I need groceries?

As you know, Governor DeWine is encouraging all individuals to remain home as much as they possibly can, especially older adults and other vulnerable groups. If you need groceries, Kroger will deliver them to your doorstep for an additional fee. If you’re interested in ordering your groceries for delivery, residents can call 782.2501 for assistance. If they need basic items like extra toilet paper, bread, eggs, and cereal, they can order from the café at 782.2444. The University of Cincinnati has also created a program that matches qualified volunteers with older adults who need assistance with groceries, prescriptions, or just someone to talk with during this time. You can call 513-327-8583 to sign up for the UC sponsored program.

What if family members want to visit?

Unfortunately, the virus spreads very easily. To prevent the spread of the virus amongst our residents and staff, we have to prohibit visitors in Assisted Living and Skilled Care Nursing. Independent living may have visitors, but we are requesting that visits are brief and only take place if absolutely necessary. We are also encouraging all necessary visitors to wear masks when visiting Independent Living.

Do I need to wear a mask?

The Centers for Disease Control and Prevention have recently suggested all individuals wear masks to help prevent the spread of the virus. Masks help infected individuals who might not have symptoms from infecting others. You are encouraged to wear a mask if you have to go out into a public space like a grocery store or pharmacy. You can make your own mask simply. To receive instructions on mask-making, call 513.909.7935.

Who do residents call if they have an emergency work-order?

As you might know, all non-emergency work-orders have been delayed due to the reassigning of many maintenance team members. If a resident has an emergency work-order, they can place it by calling 513.909.7935. Emergency work-orders will continue to be prioritized.

What should a resident do if they feel lonely or depressed?

Many older adults who are living alone and abiding by the self-quarantine mandate are at risk of loneliness and isolation. This can be especially harmful to your health. If you feel like you might be experiencing symptoms of loneliness, you might consider using a video conferencing platform, like Skype, to talk to family and friends, call your neighbors, your healthcare provider, or us at 513.909.7935.

How long is the stay-at-home mandate?

The hardest part with many of these mandates is that we aren’t sure how long they will last. Currently, the mandate is planned to last until May 1, 2020.

How is Maple Knoll regulating who enters and leaves the facility?

Maple Knoll is requiring all staff to enter through one entrance where they go through a screening process which includes taking temperatures and asking a brief series of questions. Employees are required to wear preventative masks in public spaces, such as hallways and in the Café.

How are residents staying busy?

Maple Knoll has gone virtual! TikTok, an online platform popular among millennials, allows its users to create short one minute videos featuring new dances, skits, and songs. Our Marketing and Activities teams have been busy filming TikTok videos featuring our very own residents. It’s no surprise that our videos are widely popular on the platform, brightening people’s days during these uncertain times. Other TikTok users leave comments letting us know how much the videos brighten their day as well. Some of our favorite comments are: “This is the best TikTok video ever! Their joy and energy is awesome” and “This page brings me so much joy. Thanks for doing this!” Thank you to all of our participants and be sure to check out Maple Knoll Village’s TikTok page!

Maple Knoll Communities’ top priority remains the safety of our residents and staff. We have recently implemented all staff wearing medical masks while on Maple Knoll’s campus to protect our residents. In addition, the CDC issued the recommendation that citizens wear face coverings in public settings where other social distancing measures are difficult to maintain, especially in areas of significant community-based transmission.

As shared by the CDC, we would like to note that wearing a mask will not prevent someone from contracting COVID-19, but it can help prevent them from spreading it to others — especially if they have it, but are not yet showing symptoms.

Wearing a cloth mask in public is not mandatory, but it is strongly recommended. Cloth masks are not a substitute for social distancing, but rather, it is a complement to social distancing. We have compiled a list of tips and diagrams for creating your own cloth mask at home. You can request a copy by calling 513.909.7935. Friends of Maple Knoll Communities,

we are closely monitoring the COVID-19 situation and, in accordance with The Ohio Department of Health, World Health Organization and Center for Disease Control,, are continuously evaluating our policies, procedures and taking extra precautions. Because the health and well-being of our residents and employees remains our first priority, we continue to make extensive preparations, realizing this situation has an unpredictable path. Maple Knoll Communities’ Corporate Infection Preventionist will be coordinating all precautions, screenings, supplies, education, updates and communication related to the virus. We have implemented the following procedures and responses:

• Visitors and Resident Access: ALL VISITORS are now restricted from entering all areas of Maple Knoll Communities, including Assisted Living, Skilled Nursing and Independent Living Common spaces. In addition, all individuals are asked by the State of Ohio to shelter in place.

• All essential employees, vendors and other medical personnel will be screened prior to being allowed entry to our facilities.

• Delivery of meals will be provided to our Independent Living residents at their request. Assisted Living and Skilled Nursing residents will continue to have their needs met by our staff. Residents have been given essential information on accessing meals and other necessary items. Loved ones of our residents can learn more by calling 513.782.2500. We are committed to doing whatever is necessary to ensure the essential needs of our residents continue to be met. Thank you for your patience as we continue to work through what we deem is necessary to protect those living and working in our communities.

We thought you would like to know that employees from every department have “stepped up” to assist with meal preparation and delivery, essential care, activities, cleaning and even taking the time to check in with our residents to assure they are doing well, both mentally and physically. Some of our efforts to assist these employees and residents have included:

· Providing free meals to employees during their shifts, including lunch and dinner.

· Restricting access to Independent Living Common Spaces, Assisted Living and Skilled Nursing.

· Screening of all essential employees entering the building.

· Providing onsite childcare for both Maple Knoll Village and The Knolls of Oxford. We recognize many of our employees may have challenges in finding child care during this difficult time and therefore we have just been approved by the State of Ohio to operate a temporary child care center through April 6th or until we are notified it is no longer needed or an extension is granted. This service will be FREE to employees.

· Creation of a Pandemic Pantry offering numerous essential-type items that may be difficult to obtain as well as will offer some of these things for sale in the café.

· The Employee Appreciation Team has been visiting all areas providing snacks, thank you’s and small tokens of our appreciation.

· Employees have been reassigned as needed- for example: wellness center employees providing activities in nursing, maintenance and housekeeping assisting with meals, screening and activities.

· Continuation of meals and activities in Assisted Living and Skilled Nursing by implementing new programs which allow for 6 feet of social distancing and educational opportunities to stimulate each resident.

· Assistance with Skype and Facetime for residents in all levels of care.

· Working on the creation of a Pen Pal program between MKV and TKO IL residents.

· One on one resident activities such as delivery of snacks, flowers, care packages, games and puzzles.

· Addition of 24 hour programming on our resident television station which includes movies, exercise classes, prayer groups, Ted Talks, trivia, meditation and more.

· Delivery of meals provided to those in Independent Living by request and at no additional charge.

· Resident assistance with Kroger grocery ordering and delivering via telephone.

· Offering of essential grocery items, such as bread, milk, eggs, cheese, cereal and cleaning products via door to door delivery from our café.

· Promotion of request for letters and well wishes for our residents.

· We are currently in the process of finalizing our resident family member app which will allow all family members to access information similar to our current resident app.

You can be instrumental in limiting your risk for exposure and/or spreading the virus by;

1. washing your hands with soap and water or alcohol-based hand rub/sanitizer (must be 60%+ alcohol) for as long as it takes you to sing the “Happy Birthday” song TWICE

2. covering your mouth and nose when coughing/sneezing

3. disposing of used tissues immediately (don’t leave them laying on surfaces)

4. maintaining your distance from those exhibiting symptoms

5. avoiding touching your eyes, nose and mouth

Our main sources for Coronavirus Information and Guidance continue to be The Center for Disease Control (CDC), World Health Organization (WHO) and Ohio Department of Health (ODH). Maple Knoll Communities’ Corporate Infection Preventionist will be coordinating all precautions, screenings, supplies, education, updates and communication related to the virus. Maple Knoll Communities will continue to provide all necessary operational support to respond to an outbreak, and abides by the World Health Organization and Centers for Disease Control’s standard recommendations to prevent infection spread.
